Simple steps to refer a patient to Royal Home Health. We understand the regulatory measures of making a home health care referral and we are here to assist in every way possible. We have a fast referral process and can accept referrals electronically, by fax, e-fax or phone. We can also come to you and collect necessary information and schedule a focused introduction of our services you will be needing prior to a procedure or discharge from a facility.
To help establish a high quality, comprehensive plan of care, we will request the following information upon referral:
